Title: The full title of your map
Author/s: The author/s name/s
Difficulty: A reasonable difficulty rating from 1 to 10 where 10 is hardest
Description: Describe your map, the more, the better
Episode: Your suggested episode, more info in the appropriate episode thread
Screenshots: Screens, the more the better
